We require that you verify the information about your practice and the networks you participate in at least every 90 days. View the instructions for verifying your information and notifying us of changes.

Credentialing status

We are now processing credentialing applications submitted on or before February 21, 2025. Completion of the credentialing process takes 30-60 days.
